This Policy applies to information collected through the Site only. Information collected during in-person visits, scheduled appointments, or through patient intake forms is governed separately by HIPAA and our Notice of Privacy Practices (a separate document you receive at your first appointment).

HIPAA and Patient Health Information
This Site is not used to collect, store, or transmit Protected Health Information (“PHI”). If you contact us about your medical condition via the contact form or email, you do so at your own discretion and we cannot guarantee the security of unencrypted communications. We recommend calling our office directly at 786-808-8555 for medical inquiries. When you become a patient, your medical information is protected under HIPAA. Please see our Notice of Privacy Practices, available at any Miami Wellness office.
Children
The Site is not directed to children under 13. We do not knowingly collect personal information from children. If you believe a child has provided personal information, please contact us so we can delete it.
Your Rights
